Parents must complete and sign this form for each of their students in grades 5 - 12. Students must return the form to Mrs. Greer in the receptionist’s office on the main campus on or before the first day of school.
This health form will NOT be published in the 2010-2011 Student-Parent Handbook.
The Health Form for Grades PK - 4 will be given out at the Parent Preview on August 12.
